Whether an individual has just been diagnosed with diabetes or has had diabetes for years, self- management is the key to living a healthy life. Although diabetes cannot be cured, it can be controlled through good self-management. Since 1994, the Diabetes Management Program has provided education and support to patients and the community. The program offers comprehensive classes, individualized instruction and a support group that is open to the public. Our goal is to help each patient and family develop a self-management plan that meets their individual needs. Learning the skills and strategies needed to control diabetes will help them live healthier happier lives. Call for more information.

A free educational support group open to anyone with diabetes and their family support team. Focuses on dealing with diabetes, nutrition and related health issues. Meets the second Tuesday of every month, 6 p.m. in Classrooms 1 and 2 at the Alexander Campus, 2610 Alexander Drive,
Baytown.

FOOD STAMP PROGRAM (LONE STAR CARD)

The Food Stamp program is a federally funded service that provides a supplemental food budget for low-income individuals. To find out how to access this program, eligibility requirements and where to apply, call the TEXAS FOOD STAMP HOTLINE TOLL FREE AT (877) 556-2200.

HOME-DELIVERED MEALS

Home delivered Meals-on-Wheels helps to maintain and/or improve the health status of older persons, support independence, prevent premature institutionalization, and allow early discharge from hospitals, skilled nursing facilities, or other residential care facilities. Call for information on the areas served.

Home delivered meals are available for ill and home-bound individuals 60 and over.

Delivers five frozen meals a week to needy seniors in Chambers County.

Geriatric Care Management

The purpose of geriatric care management is to assist seniors and their families to activate community services, and work with appropriate financial and legal personnel.

This organization provides a free list of geriatric care managers in your area on their website which is updated weekly; or a directory can be mailed for a $15 fee.

Health Education

Offers a variety of classes on health education and safety to promote personal health and proper care of victims of accidents, heart attack, and other emergencies.

Information includes classes available to the public and events in the community.

Health Screening and Geriatric Assessments

As you age, you may experience sudden or gradual changes in your body. Regular assessments are one of the best ways to deal with health challenges. The following contacts can be called to find times and locations for various types of health screening including blood pressure, cholesterol and diabetes.

Provides information and assistance on case management, assessment and screenings available in the community.

Home Care and Home Support Services

Housekeeping tasks, bathing and daily living tasks, meal preparation, shopping, laundry, escorted transportation, minor repairs, and companionship are offered in personal care services. For additional resources, PLEASE SEE CHAPTER 2, HOME CARE AND HOME SUPPORT SERVICES.

Home Health Care

Home health care services are available to those who have difficulty leaving home and need skilled services such as nursing or physical therapy. A physician’s order is required before providing care. Home Maintenance and Repair

Help with household management and maintenance, utility assistance, minor construction, seasonal weatherization and repair is available for health and safety needs. There are many private companies that provide multi-language translation services. Please refer to Translators/Interpreters section in your local telephone directory.

Immunizations

Don’t overlook your need to be immunized for tetanus, influenza, and pneumococcal pneumonia; or for diphtheria and measles if you’ve never been immunized for these diseases. Keep an ongoing record of immunization types and dates in an accessible location.

Offers pneumonia vaccine to seniors 65 and over. Also provides flu shots.

Medical Equipment and Supplies

Medical equipment and supplies can be costly, so it’s important to shop around and understand the benefits and limitations of your insurance or Medicare/Medicaid coverage. Sometimes you can save money by purchasing used equipment, but most equipment gets fairly heavy use, so inspect it before buying.

Upon request, this office coordinates with other agencies and individuals to obtain medical equipment for persons in need.

Donated and recycled medical equipment available to the lower-income disabled residents of Texas. Eligibility requirements include prescription on a prescription pad, proof of income for all persons living in household and a Social Security card. Donations given by appointment only.

Medical or Safety Alert

Personal, non-profit response service. Electronic pendant/wristband used to alert Lifeline Monitoring Center. Immediate contact for emergency responders (24 hours).

“EMS – No CPR” Directive

Also known as a “no code” form, this document signals emergency professionals to not perform CPR. Forms are only available from physicians’ offices, hospitals and hospice or home health organizations (for clients only).

Call for information about a bracelet identifying your medical information, as well as instructions for resuscitation, etc. Yearly membership fee and initial setup charge apply.

A nationwide identification support and registry program. Twenty-four hour assistance is provided, whether a person becomes lost locally or far from home. A lifetime protection fee of $40 is charged.

Outreach

Telephone reassurance is provided to seniors living alone. If there is no answer or if the caller suspects something is wrong, the appropriate authorities and designated contacts are called.

Books are mailed out to the elderly or homebound residents of Baytown. Can supply large print or talking books. Contact the library for more information and/or to apply for services.
